How can an individual receive Home Care services?

Depending on your needs, you can receive skilled nursing or attendant services. Once we identify your specific needs, we can recommend services to fit your unique requirements.

Who pays for Home Care?

Home Care can be paid for in many ways. Medicaid, medicare, Waivers, private health insurance, long term care insurance, private pay, etc. Exceptional Hearts accepts Private pay, OLTL and Aging Waivers.

What are Waiver services?

Home and Community-Based Services (HCBS) Waiver Programs are available in Pennsylvania to provide supportive services to qualifed individuals who wish to remain in their home and/or community rather than enter an institution (such as a nursing home or intermediate care facility).

Waiver programs provide certain medical and non-medical services to an individual in their home or in another community-based setting in order to help them remain living independently in the community (which is the preference of most individuals). The services provided under the various programs are not typically covered under Medicare and/or Medical Assistance, especially on a long-term basis.

Exceptional Hearts can assist you in obtaining these services.

What procedures do you have in place to handle emergencies?

Our personnel is available 7 days a week, 24 hours a day, to handle simple emergencies. Patients will be triaged and then, based on the type of emergency, the appropriate sta member, clinician, or paraprofessional will be assigned to do follow-up. In the event of a natural disaster, the agency has a comprehensive emergency operations plan and patients are given information on that in their pack-age.

How far in advance must I call to set up services?

Depending on the type of service you require, you can receive services the very same day (insurance or private pay) or several months (Waiver services). Once we identify your needs, we will recommend all of the possible options.
